We are 100% committed to making your visit to our office comfortable and as easy as possible.
Referral visits, what to expect:

A comprehensive dental exam, including a full mouth examination of your teeth and gums BEFORE we perform treatment

Unless absolutely necessary AND if there is time available the day of your exam appointment . We cannot guarantee treatment will be done during a consultation visit.

Any necessary low radiation digital X-Rays and 3D scanning according to your referral sheet.

An introductory conversation with your doctor, where we will listen to your needs, concerns, and expectations. Further treatment will be discussed and a following appointment will be made if needed.
What to bring on your first visit:
For your first dental office visit, please be sure to arrive 5-10 minutes before your appointment time and to bring the following information:

Your Dental Insurance card (please give our office a call if you would like to check if we accept your Dental Insurance).

Driver License/ID.

If a referral, please bring in the referral form from your dental location.
Please be aware of your:

Current medications you are taking including dosage.

Your completed Dental History

Any allergies
Important Requests

A parent or guardian must accompany all patients under 18 years of age.

Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, joint replacement, etc.).

Also please let us know if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.) OR require medication prior to dental cleanings (i.e antibiotics for pre-med.)

We understand that emergencies happen, but when a patient does not show up or cancels without at least 24 hours' notice, we are unable to offer that time to another patient in need. Advance notice allows us to manage scheduling effectively and maintain efficient clinic operations, which is especially important for health and safety protocols. Since we provide one-on-one treatments, missed appointments create a significant inconvenience for your dentist, our office, and other patients.

If you would like to fill out the forms prior to your appointment it will you some waiting time. If you have any questions or concerns, please feel free to give our office a call.
